The capabilities of mailfromd could be described as: "Absolute control upon whole SMTP session".
I really do not know (though not pretending on absolute in this) more flexible SMTP filtering tool.
It cooperates with ClamAV and SpamAssassin and does that better than native milters supplied with them, it can call system commands and run what you want for the conditions you are defining.
It light and resources friendly, it was created to resist really heavy SMTP load.
My honest opinion it is the "must have" tool for SMTP admins.
