This tool is what I've been looking for a long time. I've found informations about it on the pgcon 2008 in Ottawa.
The tool works on all platforms that support Java.
Now I use it for round about two years for designing new databases and reverse engineering existing databases. There is also a closed source version available, which I didn't need at all by now.
The project has no license, no downloads, no sources, just a link to a commercial page, where one can buy accounts for 50 bucks per user.
For me, this is everything but open source and especially it is not free. The only intention could be to spam on ohloh.net.
