If we have the same text more than once in a contents table (it could happen!) then add -2, -3 and so on to following occurances, so that all anchors are unique.

Implemented support for non-random anchors in the contents tables. In order to preserve sanity in the context of wiki's ephemeral nature, we use the compacted link text, not the heading numbers.

Don't call ::constrain() in setFullScreen(true), because many applications specify a either max-size, aspect-ratio, or some other hint that simply doesn't allow the window to cover the entire screen. Let's try to be smarter than such applications and simply cover the entire screen (since this is probably what they expect and want).
